A denied claim is defined as any claim that reports a value of 04 Denial for data element Number 02 Maintenance Type Code as defined in IAIABC Release 1.0 guide and California EDI Implementation Guide for the First and Second Reports of Injury (FROI/SROI) Version 3.1.
